Putnam County Office for Senior Resources is responsible for stimulating, promoting, coordinating and administering local programs and services for older Putnam County residents. In carrying out this responsibility, the Office for Senior Resources’ primary emphasis is on the development of long-term care programs and services necessary to meet the long term care needs of Putnam County’s elderly and to support informal caregivers.

This federally-funded program is designed to help with the payment of heating/utility bills. The Office For Senior Resources certifies applications for residents 60 years of age and older and SSI recipients of any age.

Provides limited demand response medical transportation for getting to and from doctor and hospital appointments for seniors who have no other means of transportation and who otherwise could not obtain needed medical care.

The Outreach Program assists the isolated and frail elderly in the community by obtaining services and providing them with information and referral as necessary. Outreach reaches OUT into the community to the unserved and underserved seniors of Putnam County, by assisting in certifying or re-certifying eligible seniors for programs such as Medicaid, Food Stamps, and/or the Medicare Savings Program. Outreach workers also do friendly visits and telephone reassurance. All Putnam seniors 60+ are eligible to receive a Putnam County Photo ID card. The card allows seniors to obtain discounts at area merchants and places them on the the Office For Senior Resources mailing list, enabling them to receive important information about aging issues and available programs and services. To obtain this card, fill out an application at the Office between the hours of 10am and 2pm Monday through Friday. Please bring proof of age and residency (driver’s license, Medicare card, etc.) Suggested contribution $2.00, but no one will be denied a card due to inability to contribute.

A home care program providing a variety of non-medical services to the elderly with functional impairments and who require assistance with bathing, eating, toileting, dressing, housekeeping, etc. This is a cost-sharing program based on income. Need and eligibility is determined after an assessment by a case manager.

EPIC was developed to assist eligible NY State residents 65 or older with the high cost of prescription drugs. Seniors who meet income guidelines and who are not already covered by Medicaid or other programs which supplement the cost of prescription drugs may be eligible.

The Putnam County Office for Senior Resources has a Health Insurance Information Counseling & Assistance Program (HIICAP) Coordinator to help you sort through the maze of insurance options to find the correct answers for you! HIICAP provides free, accurate and unbiased information, counseling, assistance and advocacy on Medicare, private health insurance, and related health coverage plans. Emphasizing wellness at all four friendship centers monthly by utilizing community resources such as volunteers from the American Heart Association to do blood pressure readings, local physicians to speak on health topics, Putnam County Health Dept. to give flu and pneumonia shots, and local pharmacists to answer questions concerning medications. “Please come and participate…….remember, HEALTH IS WEALTH!!”

Putnam NY Connects provides a plethora of services for individuals looking for Long Term Care, however the general service we provide are Information and Assistance, Needs Assessments and Public Education.
Information and Assistance
An objective and comprehensive information is gathered from each Putnam resident to determine the appropriate institutional service and or eligibility criteria to link them with the services and resources available to help meet their particular needs.
Public Education
In an effort to reach as many people as we can Putnam NY Connects will have on-going education and awareness campaigns in and around the community about long term care.
Needs Assessments
To ensure appropriate needs are being met a preliminary evaluation is done for all individuals to identify any social, medical and financial needs as well to determine the available service options and choices to meet these needs.

The Adult Day Services Program is designed to meet the needs of functionally impaired adults through individual plans of care. This program provides a variety of health, social and support services in a protective setting during daytime hours. To be eligible, participants must be socially isolated, able to function in an ambulatory care setting and require some assistance with activities of daily living. A friendly bus service is also available to provide transportation to and from the two Adult Day Care sites for regular day care hours if needed. Our two sites are located at the following locations: William Koehler Friendshop Center, 180 Route 6, Mahopac, NY and Putnam Valley Friendship Center, 117 Town Park Lane, Putnam Valley, NY. The hours are: 9:30 a.m. to 2:30 p.m., Monday through Friday with a suggested donation of $5.50 per day. For more information on attending this program, call (845) 808-1737.
There are four nutrition centers (open 9am to 3pm, Monday through Friday) which serve hot nutritious noontime meals, offer companionship, nutrition education, health counseling (blood pressure readings, talks by local physicians and pharmacists, etc.), transportation and shopping assistance. Transportation is offered for those who have no other transportation. Most special diets can be incorporated into meals served. Home-delivered meals are also offered (assessment by the Office For Senior Resources is required). If you would like to participate, call your nearest center or the Office For Senior Resources (845) 808-1700. Reservations should be made two days in advance.
A hot, nutritious meal is delivered directly to the homes of sick or homebound elderly. A physician’s referral and home assessment is required.

Delivers frozen meals to the homes of eligible seniors. Meals are provided for up to 7 days per week depending on individual circumstances. An assessment is required.

RSVP enables adults 55 older to contribute meaningful service in a wide variety of community agencies. Volunteers are matched with assignments of their own choosing. The Program provides orientation, training, and transportation, as well as the supplementary insurance while on duty. RSVP volunteers lead more active, dynamic lives, full of the excitement of participation.
