Executive Kevin Byrne, New York State Senator Peter Harckham and a representative from
Assemblyman Matt Slater’s office, liver transplant recipient Janet Ocasio and Executive Director of

Now when customers register a vehicle at any DMV office they will be asked if they would like to
join the organ donor registry. Utilizing the customer facing device at each DMV window. This
question will be asked before a customer completes the registration. When registering to be a part of
the Donate Life Registry, “Organ Donor” will appear on the front of their DMV photo documents.
Donate Life custom license plate is also available and can be ordered on the DMV website with $20
of the annual fee directed to the “Life Pass It On Trust Fund”, which is used for organ donation
and transplant research and educational programs.

“I, along with all of our partners who were present at today’s event am excited to continue working
together to increase organ donor enrollment through New York State,” said Putnam County Clerk
Michael Bartolotti.

There are several ways a person can enroll: The NYS Donate Life Registry Website, NYS Department of Motor Vehicles website, when completing
the donation section of the driver license or non-driver ID renewal application or when registering
to vote there is a section of the voter registration paper or online form.

“We are proud to take every opportunity to remind our customers of the great need for organ
donors. So many families and individuals have been impacted by this live-saving step, and others are
anxiously waiting for a donor organ to be available,” said DMV Commissioner Mark J.F. Schroeder.
