Physical/ Drug Screening Required / *am - 4pm Mon- Fri POSITION SUMMARY: The Social Services Assistant provides support and assistance to Sisters in all Center Communities to identify, solve and/or cope with challenges faced in their daily lives. The Social Services Assistant supports Sisters with gaining better understanding of the emotional, social and physical challenges of getting older and implements person-centered, holistic solutions to these challenges. The Assistant meets with Sisters who are current residents as well as those who are new to the Center to assess support needed. The Social Services Assistant acts as an advocate and liaison with family, care givers and service providers both internally and in the community at large. ESSENTIAL FUNCTIONS, LISTED IN PRIORITY: In collaboration with the Center Coordinators, assesses Sister's strengths, needs, and goals and works with the team to meet these goals; monitors and evaluates progress. Assists in the transition of Sisters from one Maryknoll Sisters Center Community to another by working with the Transition Team and following the transition process; coordinates and assists Sisters in moving to new living area: Documents social history. Accompanies Sisters through the physical and emotional process as changes in level of care occur. Maintain consistent and positive communication and support with Sisters while addressing their concerns and goals. KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge of and ability to relate and communicate with a diverse elderly population. Ability to work compassionately and build a rapport with frail, ill and sometimes angry Sisters in professional and caring manner. Ability to work independently and with a team to problem solve. Ability to lead groups in a focused and assertive manner. EXPERIENCE REQUIRED: 3 years in a geriatric care, social work services field. Minimum 1 year working with geriatric population in a professional setting. Must possess a valid driver's license. MINIMAL FORMAL EDUCATION REQUIRED: Bachelor's Degree in the Social Services filed - social work, psychology, sociology, social services or pastoral care
